Attention New Patients

Hours are by appointment only.   Please download the Patient Intake Form, fill it out and bring it with you to your first appointment.

If you will be using health insurance, please bring your insurance card with you, and arrive 15 minutes early to record that information before your appointment.

Your first appointment will include an extensive consultation reviewing your current health complaints, life style habits and health history.   Please have a complete list of medications and supplements you are currently taking.

Payment is due following your appointment.   Should you not be able to make your appointment, please give a 24 hour notice of your need to cancel.   If this notice is not received a $75 rescheduling fee^* will be applied to your next appointment.

Monthly Newsletter

Qi Mail is Dr. Hahn's monthly educational newsletter which focuses on various and timely topics related to acupuncture and herbology.   If you would like to be added to our email list, click here or please email Dr. Hahn or contact our office.

Payment

Dr. Hahn is an in-network provider for United, Blue Cross Blue Shield, and Presbyterian insurances.   You may also check your particular insurance provider to see if Dr. Hahn is listed as an "in-network provider."   She also accepts cash, checks, Visa and Mastercard.

Please call your insurance provider to verify acupuncture is covered in your plan.
